I recommend this recently updated article from the vast AAP library: www.americanaquariumproducts.com/Vibrio_Aeromonas.htmlHere is an excerpt: "TREATMENTS: Treatments of Aeromonas in tropical freshwater aquariums include: Pimafix AND Melafix (combined) but only for VERY MILD infections, especially mild wound infections (Melafix by itself is rarely effective on Aeromonas though since it is gram positive). Pharmaceutical grade Nitrofurazone is the starting point for moderate to serious Aeromonas infections. The product of choice is AAP Furacyn which is vastly superior to API Furan 2 (it is also pharmaceutical grade which Furan 2 is not). AAP Furacyn can be combined with Kanamycin for an even stronger combination treatment is generally my recommended default staring place for in tank or hospital tank treatment. If combining, best to use an already combined product such as AAP Spectrogram (although currently unavailable) being a more synergistic combination of Kanamycin/Nitrofurazone and is easier to use and pharmaceutical grade too. While a separate combination treatment of Kanaplex & Nitrofurazone (AAP Furacyn) can be effective too, the substitution with Furan 2 is not as effective as AAP Spectrogram or Kanaplex plus AAP Furacyn due to loss of a synergistic combination and because of different treatment schedules and lack of purity by NON pharmaceutical grade Furan 2. "

I'm reposting here an email conversation with the helpful folks at AAP before I was aware of this forum.
Dear AAP, One of my male tropical angelfish has an Aeromonas infection with skin ulceration and a small furuncle at the base of a fin. He’s lethargic and eating poorly. Conditions in the 170 gallon aquarium are pristine. I believe he became infected as a result of dominance stress with another male angel. I have had only partial success with two series of salt-methylene blue baths (5 days followed by another 5-day course), then a course of spectrogram 3 treatments every other day as recommended in an isolation tank.
I have ordered Mebromide. The topical application will be stressful for the fish and will not address the furuncle. I would very much appreciate your advice at this point. Should I change antibiotic? Another course of spectrogram?

You note pristine water conditions, but could you provide all parameters here in our customer service email or our sponsored Everything Aquatic forum? Aeromonas is often water parameters issue, even when the main cause is from another fish as otherwise the wound should heal with minimum effort
AAP Wound Control when applied carefully should not have any undue stress to the fish. The problem is usually when the person treating is struggling to do this, but based on your profession, this should not be an issue
A full bath with added medications such as AAP Bettamax might also be helpful as with this problem. Reference: Fish Baths
SeaChem Stressguard in the holding tank might also be helpful along with AAP Quinex after a bath every other day or water change
And yes, another regimen of Spectrogram might be advisable (although we are currently out of this product) [My batch was purchased in 2018]

Welcome to Everything Aquatic! Thanks for posting parameters which look good and further makes a serious Aeromonas infection less common (but not impossible) If you could share a magnified picture, this might be helpful (ProBoards is at its limit, but you can host the picture elsewhere, then share). Make sure to separate this fish to reduce stress and help prevent further aggressive behavior. A net breeder box might be useful so as to keep the fish in the same tank if no hospital tank is available Otherwise I would follow the advice given already

Have you ever measured pH through out the day and a time at night? Co2 diffusion can effect pH driving it down through the day and swinging back up at night. I would suggest multiple readings throughout a 24 hr cycle. Your KH looks good, but may not be able to hold the pH stable. Just a thought as a stressor along with the fish bullying.
I agree a picture would be helpful.
Are you also using a GH replenishment? I recommend AAP wonder shells used for the life of the tank. Keep the electrolytes up to keep immunity boosted for all fish. Would also help more during the treatment process.
The sore may be a secondary issue to a effected immunity.
Hope all this helps! I know angles can be rough on each other as they fight for territory.

Thank you Carl and Devon. pH was measured in the evening after a full day of CO2. I suspect it's higher in the AM. My GH is already high (we have well water here in Vermont) and I'd be reluctant to increase it further. The 8 other angels in this 170 gallon aquarium are fine, and I do think that dominance stress is the cause of the opportunistic infection in this male. I have some photos - could send via email if you wish. I'll post an update once I try your other suggestions along with another course of Spectrogram (purchased in 2018).
