Thanks, Devon and Carl. I brought the fish to an aquatic vet, who believes the problem to be mycobacteriosis with no cure. I plan to take a break from fish keeping after this, but I really appreciate all the help you've given me. Sorry to read about your break. Did the vet take a stain, as mycobacteriosis is easy to identify with a stain and test which he should be able to do. I was told that there was not an antemortem test available for mycobacteriosis, and I wasn't willing to euthanize the fish at the time because she still has a pretty high quality of life. If/when the fish (Angelfish D) dies, I might have them perform a necropsy to find out for sure.
Angelfish D started hiding and staying near the top of the tank a few weeks ago, like two of the fish that died before her. The first symptoms for Angelfish A appeared in October (or maybe even back in July when the fish were received), and Angelfish A died February 28th. Angelfish B started hiding and acting lethargic in February or March and died on May 9th. Angelfish C did not fit this pattern and died suddenly in April without obvious symptoms beforehand. The vet didn't notice any visible symptoms in the fish I brought in besides some trouble swimming, and did not think parasites were the cause. Definitely a frustrating situation.

Again, I am very sorry this has happened, if this is indeed Angelfish Virus, this is a very frustraing an nearly impossible disease to treat.
This said, your most recent description clearly does NOT fit that for mycobacteriosis as per the noted article I shared earlier

I had a necropsy performed and mycobacteriosis was confirmed as the cause of death. Wish I had a better update.
