Definitely some pop-eye, but no obvious infection which is very good. The usual reason is build up fluids behind the eye, although the causes can vary A bath or often a dip (which is shorter duration than a bath; 3-5 minuted at much higher sodium chloride concentrations) is helpful for this. Here is the problem with the neon, fungus on mouth, no idea how long it has been there since he's (?) been hiding in the back and out of sight until I started the Paraguard treatment for the pop eye last week. He got into Macro lens range today, so I grabbed some images of something really odd going on with his mouth, he can still eat, but seeing has to be an issue for him. I'll be continuing Paraguard until this stuff goes away, unsure if it is related to molly lesion and pop eye or not: As usual, click to enlarge... This looks like Lymposystis, which is viral. It could also be a tumor, but it if it sprung up "overnight" more likely the former While baths using usnea may help, often just keeping optimum parameters and maybe a MB bath is all need be done. I doubt this sprung up overnight, in a few other pictures it looks worn and "regrown", if that makes sense. He's in the same tank as the Molly, which has many babies and another neon, none of which are showing anything like this.
I'll see if it is larger or smaller after the second week of Paraguard treatment. Maybe I'll have a nice collection of messed up fish photos for you to use soon, if nothing else.

Today, Both of her eyes are popping out again, and she's hanging out about 20 degrees nose up/tail down around 1/2" from the surface. Other tank denizens are fine and going about their business.
Water parameters measured near optimal (outside of ORP) after daily 10% water changes prior to paraguard addition.
She looked all better yesterday. Wonder if she'll look fine tomorrow, or I can get you guys a dual-bug eyed Molly picture to add to the collection...

Since your parameters are likely as good as anyone could keep an aquarium, and since this is likely an osmoregulation issue that seems to be down, then up, then down again, I suspect your molly is struggling with age induced osmoregulation issues. This does not mean a bath or even a dip may not help (using NaCl and maybe MgSO4), but I think results may questionable.
Mollies only live a few years and often as with many fish sold commercially, when you purchase a fish, there is no knowing whether one is purchasing a young adult or breeder stock that is now being sold off to the retailer (this is common with liveberarers & Bettas).

Survivor's gill plate is pretty much healed/scarred, and her eyes are back to normal.
The Neon had no improvement after 2 weeks of Paraguard. He's still eating, so he's just a tad heavy in the bow/front.

He's an adult sized neon, not quite 1" nose to tip of tail. The pictures make it look a lot larger than it appears to the eye while he's in the tank.
It's hard to see due to the grey coloration, except when he's at a certain angle to the light in the tank. I'm nearly positive I'd do more damage to him trying to remove it than he's suffering from it. He's still active and eating, not hiding. If he starts to be a recluse, then I'll think about surgery, but I think my #13 scalpel blades may be too large. I'll have to order some of those glass chip scalpels like they use for eye surgeries.

Hi troybtj, One of my neon has what appears to be the start of this condition and was thinking of treating with Paraguard. How did you go, did you had to remove it surgically? Its looks like a tiny gel/ fluid filled cylindrical protrusion about a couple of millimeters high. It's white towards the base while rest seems to be clear. All the fish, including him are behaving and eating normally. My daughter unknowingly unplugged the heater to, put her tab on charging, the night before and the tank dropped to 19°C by the time I rectified the situation.

The latest update: The Neon status doesn't matter.
I moved about 50 mollies to the LFS to sell, along with some plant cuttings, leaving only Survivor (Molly), and the two Neons. 1 week later, the 2 Neons appear to have been consumed, along with a juvenile Male Molly I put in with Survivor.
I think she likes the aquarium to herself, or that's the message she's sending...
No big loss. Survivor is all healthy, though!
